CONSENT to the processing of personal data

In accordance with the Law of Ukraine "On Personal Data Protection" I agree to the processing of my personal data for possible further use in the organization of criminal prosecution of war crimes in accordance with Ukrainian law and the International Criminal Court in The Hague to the following extent:

• information about my last name, first name, patronymic, passport data, data on registered or actual residence, telephone numbers;
• use of personal data, which involves the actions of the owner of the database to process this data, including the use of personal data in accordance with their professional or official or employment responsibilities, actions to protect them, as well as actions to grant partial or full right to process personal data other subjects of relations related to personal data (Article 10 of this Law);
• dissemination of personal data, which involves the actions of the owner of the personal database on the transfer of information about an individual from the personal database (Article 14 of this Law);
• access to personal data of third parties, which determines the actions of the owner of the personal database in case of a request from a third party for access to personal data, including the procedure for access of personal data to information about himself (Article 16 of this Law)

In the event of a change in my personal data, I undertake to provide as soon as possible updated, accurate information and originals of relevant documents to update my personal data.
